After School/ Weekend Program:

 

Sites with an extracurricular or after school or weekend program can apply for snack and dinner free for the entire site depending upon the site location.

 

After School REQUIREMENTS:

 

§  501(c) (3) document (non-profit status)

 

§  Current Health or Fire Inspection Certificate

 

§  Completed After School Food Service Application 2010-2011 and After School Enrollment list

 

§  Attend a Mandatory Training. This is an approximately 2hr session with instructions concerning filling out the After School meal counts, lists, food amounts and handling. This is required by the State.

 

State Approval by the Dept. of Education.  This occurs once all the paperwork has been submitted. This process can take from one week to several weeks depending on the states busy level.
